At Five Health, we bring health education, prevention, and care directly into the community, making it accessible, stigma-free, and grounded in trust. Our programs address the social and structural factors that shape health, meeting people where they are and empowering them with knowledge, tools, and connections to care.
Five Health’s Home Access Program makes it easy for individuals to take control of their sexual health from the privacy and comfort of home. Through this initiative, we offer free or low-cost HIV self-test kits that can be discreetly mailed to individuals anywhere in our service area.
Each kit includes clear, step-by-step instructions and a direct link to virtual support from Five Health staff for results counseling, risk-reduction education, and linkage to PrEP, treatment, or other services as needed.
By meeting people where they are – literally – this program removes barriers such as transportation, stigma, and limited clinic hours, empowering individuals to know their status, protect their health, and connect to care on their own terms.
Healthy communities make healthy people. Through our CAP Initiative, Five Health partners with local businesses, like barbershops, bars, restaurants, nightclubs, and community centers, to expand access to essential sexual health resources.
Each partner location displays Five Health branded educational materials and provides free condoms and prevention kits in easily accessible areas such as restrooms, counters, and waiting spaces. By embedding these resources into everyday environments, CAP helps people access information and protection in spaces they trust and visit most.
Trusted relationships are the foundation of healthy communities. Through the Bridge Builders Program, Five Health trains barbers, stylists, faith leaders, and neighborhood advocates to serve as Community Health Workers — trusted messengers who connect people to the information, support, and care they need.
Bridge Builders share accurate, judgment-free education about sexual health, consent, and prevention while helping community members access services virtually or in person through our network of partner organizations. This program closes the gap between communities and care –one conversation, one connection, and one community at a time.
Every quarter, Five Health hosts Community Care Days in collaboration with trusted community pillars, churches, schools, neighborhood associations, local businesses, and cultural organizations, bringing health directly to where people live, work, and gather.
Through our network of partners, participants can receive free health screenings, vaccinations, HIV and STD testing, nutritious food, fitness and wellness activities, family support services, and on-site enrollment assistance for insurance and prevention programs.
Each event transforms familiar spaces into welcoming hubs for learning, connection, and care. By uniting multiple resources under one roof, Five Health helps communities overcome barriers to care and build lasting habits of wellness.
Through STRIDE Academy, aimed at Strengthening Training for Responsive, Inclusive, and Diverse Environments, Five Health partners with clinics and healthcare organizations to strengthen their capacity to deliver care that is stigma-free, culturally competent, and accessible to all.
We collaborate with providers to review their standards of care, identify gaps, and implement practices that build patient trust and improve health outcomes. Our tailored trainings teach evidence-based strategies to reduce stigma and enhance quality of care, covering topics such as opt-out testing, integrated screening, PrEP and nPEP, and stigma free risk assessments for HIV and other STDs.
The Academy also provides education on offering sexual health care that affirms racial, cultural, social, religious, and sexual diversity, while connecting partners to digital tools that link individuals to local testing, prevention, and treatment.
