Community Care Days: What Happens When Health Comes to Your Block

Recently, Five Health set up in a church parking lot in Liberty City. We brought free health screenings, resource navigators, hygiene supplies, and a table full of information about local services. Over 80 people came through in four hours.

That’s what a Community Care Day looks like.

More Than a Health Fair

Traditional health fairs are fine, but they tend to attract people who already prioritize their health. Our Community Care Days go to the neighborhoods where access gaps are widest. We partner with churches, community centers, and local businesses to set up in places people already trust.

We don’t just hand out pamphlets. Each event includes one-on-one conversations with resource navigators who help people identify what they need and connect them directly to services. Need a primary care doctor? We’ll help you find one. Behind on rent? We know who to call.

Why Location Matters

Research on health behavior is clear: convenience drives engagement. People are more likely to get screened, ask questions, and follow through on referrals when the service is in their neighborhood, during hours that work for them, and run by people who look like them.

That’s by design. Our outreach team is from the communities we serve. They speak the languages, know the cultural context, and understand the specific barriers that keep people from getting care.
