Onboarding, staff training, and access to the navigation network are provided as part of the partnership. There is no fee charged to the clinic for participation.

What kind of resources do you navigate patients to?

We connect patients to a wide range of community programs, social-determinant resources (like transportation or housing support), and public-health education.

Is this a referral program?

No. Five Health is a public-health organization. Participation is not tied to referral volume or value. Our focus is on patient education and community navigation.
