Our Partnership Ecosystem
Five Health works with five primary partner categories:
- Healthcare Systems
We partner with local hospitals and FQHCs (Federally Qualified Health Centers) to provide wraparound services for their patients. When a clinic identifies a patient struggling with housing instability or food insecurity, they refer to Five Health for navigation and support.
Example: Our partnership with Jackson Health System allows us to receive warm handoffs for patients who test positive for HIV or Hepatitis C, ensuring immediate linkage to treatment and support services.
- Community-Based Organizations
We collaborate with churches, neighborhood associations, and grassroots organizations that have deep community trust. They host our Community Care Days, refer residents to our programs, and provide cultural context that informs our approach.
Example: Our partnership with New Birth Baptist Church in Liberty City allows us to reach congregation members who would never walk into a health clinic but trust their church to connect them with services.
- Government Agencies
We work with Miami-Dade County Health Department, housing authorities, and workforce development agencies to align services and reduce duplication.
Example: Our collaboration with the county’s Ryan White Program ensures that HIV-positive individuals we serve can access comprehensive care coordination and medication assistance.
- Foundations and Funders
Strategic philanthropic partners provide the flexible funding that allows us to pilot new programs, respond to emerging community needs, and sustain core operations.
- Academic Institutions
Partnerships with local universities provide research support, program evaluation, intern pipelines, and evidence-based practice guidance.
