At the midpoint of 2026, Five Health has served more than 4,200 unique individuals across Miami-Dade County. This report highlights key outcomes, lessons learned, and strategic adjustments for the second half of the year.

By the Numbers (January-June 2026)

Health Access & Quality:

• 1,847 STI testing kits distributed (Home Access Program)
• 91% of kits returned for processing
• 78% linkage-to-care rate for positive results
• 412 individuals educated about PrEP; 89 initiated treatment
• 6 Community Care Days hosted, reaching 634 individuals

Economic Stability:

• 48 STRIDE Academy participants (2 cohorts)
• 81% employment rate within 90 days of graduation
• Average wage increase: $4.80/hour
• 71% enrolled in employer-sponsored health insurance

Community Engagement:

• 32 Bridge Builders trained
• Estimated 2,100+ indirect education contacts through ambassadors
• 14 community partnerships formalized

Education:

• 1,847 unique Community Connect app users
• 2,419 resource searches conducted
• 892 confirmed referrals completed

Overall Reach:

• 4,218 unique individuals served directly
• Estimated 8,500+ reached through education and outreach

Key Outcomes

Health Outcomes:

• 94% of individuals receiving navigation support successfully connected to primary care within 30 days
• 86% of chronic disease patients reported improved medication adherence after 3-month case management
• Zero new HIV transmissions among PrEP users (gold standard)

Economic Outcomes:

• STRIDE graduates earned a cumulative additional $412,000 in wages compared to pre-program employment
• 34% of participants secured health insurance for the first time

Systems Outcomes:

• Partnership with Jackson Health reduced ER utilization among super-utilizers by 23%
• Collaboration with Miami-Dade Schools connected 127 students to primary care

What We Learned

Success: Our home-based STI testing model dramatically increases testing rates. We’ll expand capacity in Q3.

Challenge: Follow-up completion drops after initial contact. We’re implementing automated text reminders and peer follow-up calls.

Success: STRIDE Academy waitlist consistently full, demonstrating demand. We’re adding a third annual cohort in 2027.

Challenge: Transportation remains a significant barrier. We’re piloting rideshare vouchers for medical appointments.

Looking Ahead (Q3-Q4 2026)

Programmatic Priorities:

• Launch pilot mobile health unit (target: October)
• Expand Bridge Builders to Broward County
• Implement chronic disease management program
• Enhance Community Connect with telehealth integration

Partnership Development:

• Formalize MOU with University of Miami for evaluation support
• Expand hospital partnerships to include Nicklaus Children’s
• Develop corporate partnership pipeline

Sustainability:

• Diversify revenue through healthcare system contracts
• Launch year-end giving campaign
• Submit applications for multi-year federal funding
