What We Measure
Our metrics framework aligns with our five-pillar model, tracking both outputs (what we do) and outcomes (what changes as a result).
- Economic Stability
- Outputs: STRIDE Academy participants enrolled and graduated; financial literacy workshops conducted
- Outcomes: Employment rate post-program; average wage increase; percentage enrolled in employer-sponsored health insurance
- Health Access and Quality
- Outputs: STI testing kits distributed; PrEP education sessions delivered; individuals screened at Community Care Days
- Outcomes: Percentage who receive test results; linkage-to-care rate for positive tests; PrEP initiation rate among eligible individuals
- Community Engagement
- Outputs: Community Care Days hosted; attendees served; Bridge Builders trained
- Outcomes: Repeat attendance rates; community trust indicators; peer education reach
- Education Access and Quality
- Outputs: Educational materials distributed; workshops conducted; Community Connect users

How We Collect Data
We use a combination of:
- Digital intake forms via Community Connect
- Post-service surveys (via text, email, or phone)
- Administrative data from partner organizations
- Longitudinal follow-up at 3, 6, and 12 months post-service
- Community health indicators from public data sources
All data collection follows HIPAA compliance standards and informed consent protocols.
Current Outcomes Snapshot (Q1 2026)
- 2,847 unique individuals served
- 89% of Home Access Program users received test results within 7 days
- 79% of those testing positive for STIs completed treatment
- 324 resource referrals made through Community Connect
- 67% confirmed successful connection to referred services
- 83% of STRIDE Academy graduates employed within 90 days
What We’re Learning
Data reveals both successes and areas for improvement. For example:
Success: Our STI treatment completion rate (79%) significantly exceeds the national average (60%), suggesting our home-based model reduces barriers.
Challenge: Follow-up completion rates drop significantly after 6 months, indicating we need stronger long-term engagement strategies.
Success: STRIDE Academy waitlist consistently full, demonstrating demand. We’re adding a third annual cohort in 2027.
Challenge: Transportation remains a significant barrier. We’re piloting rideshare vouchers for medical appointments.
Looking Ahead
We’re investing in:
- Integration with partner EHR (Electronic Health Record) systems for seamless data sharing
- Expanded longitudinal tracking to measure sustained behavior change
- Community-defined success metrics that go beyond traditional health indicators
