Five Health is expanding telehealth options for our community.

What Is Telehealth?

Telehealth lets you see a doctor or nurse from home using a smartphone, tablet, or computer. You discuss symptoms, get medical advice, and sometimes receive prescriptions—all without leaving your house.

What Telehealth Can Treat

  • Colds, flu, and minor infections
  • UTIs and yeast infections
  • Skin rashes and mild allergic reactions
  • Mental health counseling and medication management
  • Chronic disease follow-ups (diabetes, high blood pressure)
  • STI consultations and prescription refills
  • Birth control prescriptions

What Requires In-Person Care

  • Emergencies (chest pain, severe bleeding, difficulty breathing)
  • Physical exams for school or work
  • Procedures (stitches, injections)
  • Some diagnostic tests (X-rays, blood draws)

How to Access Telehealth Through Five Health

  1. Schedule: Call 305-354-9295 or book through Community Connect app
  2. Pre-Visit: You’ll receive a link via text or email
  3. Visit: Click the link at appointment time; you’ll connect with a provider via video
  4. Follow-Up: Prescriptions sent to your pharmacy; follow-up scheduled if needed

Privacy and Security

Telehealth platforms are HIPAA-compliant, meaning your medical information is protected. Use a private space for your appointment.

When to Use Telehealth vs. In-Person Care

Use telehealth for: routine issues, follow-ups, prescription refills, mental health

Go in-person for: first-time visits with a new provider, physical exams, anything requiring hands-on assessment
